Collection, Processing and Use of Personal Information and Statement of Consent
Notice of Cathay Hospitality Management (hereinafter referred to as “AMAZZING CLUB”) on the Collection, Processing and Use of Personal Information and Statement of Consent
The Undersigned and the legal representative/responsible person/auxiliary support assistant/authorized agent of the Undersigned acknowledges and agrees with the AMAZZING CLUB in the collection, processing, or use of personal information and the purposes and usage thereof, with notification of the following in accordance with the personal data protection act applicable in the relevant jurisdiction:
1.The purpose of collection: Collecting, processing and use of personal information on members within the scope of marketing and rendering of services for the members and, through the integration of information for releasing timely information on hotel rooms, foods and beverages and related information, invitation to events and marketing. The information will be the reference for rendering subsequent services to members.
2.Types of personal information for collection: Name, ID card number or passport number, e-mail account and method of contact. Please refer to the membership page of AMAZZING CLUB for details.
3.The duration, regions, targets and means for the use of personal information:
• Duration: During the period in which the purpose for collection of personal information is existing until the AMAZZING CLUB acts actively, or in ways agreed to by the Undersigned or the legal representative/responsible party/auxiliary support assistant/authorized agent of the Undersigned in writing, or in the means agreed by the Company, to request the Company to suspend the collection, processing or use of personal information.
• Regions: The locations of AMAZZING CLUB and institutions of outsourced services and of the institutions in business relations with AMAZZING CLUB.
• Targets: AMASSING CLUB and institutions in cooperation with AMAZZING CLUB or institutions providing outsourcing services to AMAZZING CLUB for marketing and providing services to members (hereinafter referred to as AMAZZING CLUB and partner institutions).
• Means: Through automated devices or other means beyond automation (hard copy or electronic file).
4.Pursuant to Article 3 of the “Personal Data Protection Act” of the “ROC” or personal data protection act applicable in the relevant jurisdiction, the Undersigned and the legal representative/responsible person/auxiliary support assistant/authorized agent of the Undersigned may exercise the following rights in writing or via the means as agreed to by AMAZZING CLUB and partner institutions with respect to the personal information retained by AMAZZING CLUB and partner institutions:

• Search, view, or request for copies. AMAZZING CLUB may charge the relevant costs as provided by law.
• Request for supplementary information or correction, provided that the Undersigned and the legal representative/responsible person/auxiliary support assistant/authorized agent of the Undersigned shall provide appropriate explanation as required by law.
• Request to stop collecting, processing or using and to delete personal information except the part of information prescribed by law or required by AMAZZING CLUB for business operations.
